I'm posting the attachments below on behalf of Saira Sidi of HealthEast.

She says:
"...here are the tools we have created at HE with the clinic patients. We have made poster specifically to have with where Dr Johnson rooms her patients. The flyer have been placed around the clinics to encourage patients to ask. The script that is used is in the third document with the process.

I have probably sent you a copy of our card that we hand out to patients, but am including it again. Feel free to post these documents on the forum."
